Initial Visit

• All initial first visits are 60 to 90 minutes

  • During this visit, Kerry will go through your medical history asking detailed questions about your health concerns, perform a personalized physical exam, discuss any recommended lab testing, and introduce you to the beginning of your treatment plan.

  • We request that you bring the following with you to your first visit:

    • Completed intake forms

    • Consent forms

    • Existing medications and supplements (containers or pictures of the containers)

    • Copy of your most recent lab work and imaging (if available)

60 Minute Visit

• The first follow up will be 60 minutes. 

• During this visit, we will review all test results from the first visit and homework.

• We will build on the treatment plan that was initiated within our first visit.
